with new business applications, where applicable.

Copy of Quote - MI quotes must also include the Application
Supplemental Information. |

Clue/MVR
Reports - If you are authorized to run them through Wisconsin
Mutual. |

Residential Cost Estimate |

By submitting a copy of the quote it will help
determine any discrepancies in premium and help eliminate policies being
issued differently than your quoted rate.
By submitting Clue/MVR reports the application
will be expedited immediately.
If your agency is manually filling out an
application, please be specific on the company plan. |
