Application Process/Requirements -

There has been an increasing amount of new
business applications coming in with missing required documents.
This is causing a delay in processing and turn around.
We are asking that all documentation be
supplied at the same time as the application, whether it is being mailed
or uploaded.
Underwriting requires the following documents
when applicable:
EFT Policies:
EFT Form
Auto:
MVR - if run in your agency
CLUE - if run in your agency
On stated value/antique vehicles - photos of
vehicle or appraisal to confirm value
** If you are authorized to run the reports on our behalf and do so,
but do not send them in, there will be a charge to the agency for
running them a second time in house.
** If you are authorized and do not run the reports, please advise.
Home:
Photos
RCE
Appraisals (jewelry, firearms, fine arts
etc.)
Suburban Supplement if farm buildings on premises/requesting the WML-503
endorsement
Commercial Auto:
MVR - if run in your agency
CLUE - if run in your agency
Artisan:
Contractors Supplement - Accord Form, so we
have a better idea of the insured's specific operations
Photos - if insuring a structure/building
BIZ:
Photos - if insuring a structure/building
RCE - if insuring a structure/building
Farm:
Photos of dwelling and all outbuildings on
premises
RCE for dwelling
Diagram/layout of the farm property is
helpful
If, in exceptional circumstances, any of these
documents are not being sent in with the application, the EFT form should
be sent directly to accounting referencing Insured Name, and the remaining
documents sent directly to your underwriter.
